Smile Lines & Nasolabial Contouring
Restoring mid-face volume and smoothing structural creases.
Our Approach
While smile lines are a natural reflection of a life well-lived, deepened creases around the mouth can cast shadows that prematurely age the face and create a persistently fatigued appearance. Commonly known as nasolabial folds (running from the corners of the nose to the mouth) and marionette lines (extending downwards from the corners of the lips), these lines are rarely just surface-level wrinkles. At Pilot Medical Clinic, we approach these creases as symptoms of underlying structural shifts. Rather than simply "filling in" the lines, our goal is to address the root cause of the laxity, restoring balanced volume and a smooth, refreshed mid-face contour.
Causes Smile and Laugh Lines
Deepened smile and laugh lines are primarily the result of cumulative anatomical changes rather than just repetitive facial expressions. Effective rejuvenation requires an understanding of these multi-layered shifts:
Mid-Face Volume Loss: As we age, the deep fat pads in our cheeks begin to deplete and descend. Without this structural support, the mid-face tissue sags downward, accumulating above the anchor points around the mouth and deepening the nasolabial folds.
Bone Resorption: Gradual bone loss in the maxilla (upper jaw) and mandible (lower jaw) reduces the architectural scaffolding that keeps the lower face taut.
Collagen & Elastin Depletion: The intrinsic breakdown of structural proteins thins the dermal layer, making the skin more susceptible to creasing during dynamic facial movements.
Ligament Laxity: The retaining ligaments that hold facial fat in place weaken over time, allowing for the downward migration of tissue.
Our Clinical Approach at Pilot Medical Clinic
Under the expert guidance of our Medical Director, Dr. Jason Lau, our non-surgical protocols for smile lines are highly customized. We focus on lifting the descended tissue and precisely restoring depleted volume to create a natural, unexaggerated result.
Our core therapeutic modalities include:
Strategic Dermal Fillers: Utilizing advanced, cohesive hyaluronic acid fillers. Instead of just injecting directly into the crease (which can result in a heavy, unnatural look), we strategically place fillers in the mid-face and cheeks to lift the sagging tissue, naturally softening the nasolabial folds from above.
High-Intensity Focused Ultrasound (HIFU): A powerful, non-invasive technology that delivers focused energy deep into the SMAS layer (the structural layer targeted in a surgical facelift). This stimulates deep tissue contraction and collagen production, lifting the cheeks and reducing the skin laxity that exacerbates smile lines.
Collagen Biostimulators: The precise injection of bio-stimulating agents (such as Calcium Hydroxylapatite or Poly-L-Lactic Acid) into the deeper dermal planes. These do not just provide instant volume; they actively stimulate your body's own fibroblasts to lay down a new collagen framework, providing long-term structural support.
Bio-Remodeling Injectables: High-concentration, ultra-pure hyaluronic acid treatments designed to flow beneath the skin, deeply hydrating the tissue and remodeling laxity to smooth out finer, dynamic laugh lines around the perioral area.

Any Questions?
-
Our foundational philosophy at Pilot Medical Clinic is to restore, not alter. By addressing the structural volume loss in the mid-face rather than over-filling the fold itself, we ensure your facial expressions remain completely natural and your smile remains authentically yours.
-
The longevity of your results depends on the specific type of filler used, the depth of the placement, and your body's individual metabolism. Generally, premium hyaluronic acid fillers used for mid-face lifting and nasolabial contouring last between 12 to 18 months.
-
While topical medical-grade skincare is essential for skin quality, barrier health, and surface hydration, creams cannot penetrate deeply enough to restore lost volume, lift sagging fat pads, or reverse bone resorption. Clinical intervention is required to effectively address the structural causes of deep folds.
-
Most of our contouring and lifting treatments involve minimal to no downtime. Injectable treatments may cause slight, transient swelling or minor bruising at the injection sites, which typically resolves within a few days and can be easily concealed. You can generally return to your daily routine immediately.
Restore Your Mid-Face Architecture
